Below is an alphabetical listing of services provided by VNA Health Care. One or more of these services may be exactly what you or a family member needs to remain comfortably at home.

Cardiac Care

VNA Health Care In-home Cardiac Care program provides treatment, education and rehabilitation care for patients recovering from cardiopulmonary disease. Together with the patient’s physician, a team of cardiac nurses implements a care plan based on each patient’s specific health needs and individual circumstances designed to achieve the highest level of wellness and functioning and reduce the likelihood of an emergency room visit or hospital admission.

Geriatric Care Management

A VNA Health Care Nurse Care Manager conducts a comprehensive, personal assessment and then develops a plan of care designed to manage and identify all the services an elderly client will need to maintain a healthy and independent life at home.

Home Care Aide

Specially trained and certified staff members carry out a personal care plan that has been developed by a physician and care manager. They will monitor patients’ vital signs and progress under the guidance of a nurse supervisor.

Meals on Wheels / Catered With Care

Nutritionally balanced ready-to-eat meals for each day of the year are delivered right to your door. Meals can be specially prepared to meet medical or dietary requirements.

Home Health Telemonitoring

The telemonitoring program gives concerned patients and family members the peace of mind that nurses are watching their condition around the clock. The program uses an in-home telemonitoring system to monitor patients’ vital signs and other health indicators. A nurse will intervene at the first sign of health abnormalities and thus reduce the likelihood that patients will need emergency room care or hospitalization.

Homemakers

VNA Health Care Homemakers are specialists who can help with cleaning, shopping, running errands, preparing meals and more. Services are available to working professionals as well as those who are chronically or acutely ill.

Hospice

Palliative care is care that is given to ease the symptoms of an illness or condition. VNA Health Care offers a network of palliative services for children and adults providing comfort and maintaining independence and dignity during terminal or life threatening illness. Psychological and spiritual support programs are also provided for family members, caregivers and loved ones.

Lifeline Emergency Response System

VNA Health Care offers an in-home personal emergency response system that ensures that older adults or people with special needs living in their homes get quick assistance whenever it is needed 24 hours a day, 365 days a year.

Medical Social Workers

VNA Health Care has a staff of skilled professionals who can assist patients and their loved ones with health, psychological, and social issues.

Nutrition Services

Registered dietitians can assess diet and nutritional health and can instruct patients on how to include new dietary recommendations into their lifestyle.

Personal Care Attendants

VNA Health Care Personal Care Attendants (PCA) can help with activities of daily living such as shopping, bathing, dressing, meal preparation, eating, grooming, housekeeping and day-to-day needs. PCAs can also assist with therapeutic exercise. They can be available for as little or as long as needed up to 24 hours a day, seven days a week

Private Duty Nurses

When patients’ health conditions require professional nursing care, licensed or registered nurses who specialize in home care are available for four hours or longer, up to 24 hours a day.

Rehabilitation

Registered and/or licensed health professionals help patients of all ages with speech, physical, and occupational therapies using a care plan specifically customized for each patient. Rehabilitation therapies are designed to improve the ability to accomplish daily activities.

Visiting Nurses

Registered or licensed practical nurses work in the home environment providing skilled nursing services. Specialties include: disease management, behavioral health, maternal and infant health, pediatrics, geriatrics, oncology, cardiology, diabetes, telemonitoring, wound care and other health-related fields.

Volunteer Social Support

VNA Health Care matches patients with volunteers who supplement the work of professionals and support staff by visiting as a Friendly Visitor, a Caring Neighbor, Hospice volunteer or Bereavement supporter. Additional caregiver support and group support meetings are also available.

Wellness Clinics and Education Programs

VNA Health Care reaches out into our communities with a variety of adult and senior clinics offered throughout the state including blood pressure, cholesterol and diabetes screenings; nutrition and development counseling; and flu immunizations. VNA also offers caregiver and bereavement support groups as well as health education seminars for the public.
