VNA Health Care In-home Cardiac Care program provides treatment, education and rehabilitation if you are recovering from cardiopulmonary disease. A VNA Health Care Nurse Care Manager conducts a comprehensive, personal assessment and then develops a plan of care designed to manage and identify all the services that you will need to maintain a healthy and independent life at home. Home Care Aide

Meals on Wheels / Catered With Care

Nutritionally balanced ready-to-eat meals for each day of the year are delivered right to your door. Meals can be specially prepared to meet your medical or dietary requirements. Read more about the independent living services we have to offer.

Home Health Telemonitoring

The telemonitoring program gives you and family members the peace of mind that nurses are watching your condition around the clock. The program uses an in-home telemonitoring system to monitor your vital signs and other health indicators. A nurse will intervene at the first sign of health concerns and thus reduce the likelihood that you will need emergency room care or hospitalization. Homemakers

VNA Health Care Homemakers are specialists who can help with cleaning, shopping, running errands, preparing meals, and more. Services are available to working professionals as well as those who are chronically or acutely ill. Read more about the independent living services we have to offer.

Hospice

Palliative care is care that is given to ease the symptoms of an illness or condition. VNA Health Care offers a network of palliative services for children and adults providing comfort and maintaining independence and dignity during terminal or life threatening illness. LifeLine Personal Emergency Response System (PERS)

Medical Social Workers

VNA Health Care has a staff of skilled professionals who can assist you and your loved ones with health, psychological and social issues.

Nutrition Services

Registered dietitians can assess your diet and nutritional health and can instruct you on how best to include new dietary recommendations into your lifestyle.

Personal Care Attendants

VNA Health Care Personal Care Attendants (PCA) can take care of your daily grooming, housekeeping and day-to-day needs. PCAs can assist with therapeutic exercise, shopping, bathing, dressing, meal preparation and eating. They can be available for as little or as long as needed up to 24 hours a day, seven days a week. Private Duty Nurses

When your health conditions require professional nursing care, licensed or registered nurses who specialize in home care are available for four hours or longer, up to 24 hours a day. Read more about the independent living services we have to offer.

Rehabilitation

Registered and/or licensed health professionals help people of all ages with speech, physical, and occupational therapies using a care plan specifically customized for you. Rehabilitation therapies are designed to improve your ability to accomplish daily activities.

Visiting Nurses

Registered or licensed practical nurses work in your home environment providing skilled nursing services. Specialties include: disease management, behavioral health, maternal and infant health, pediatrics, geriatrics, oncology, cardiology, diabetes, telemonitoring, wound care and other health-related fields. Volunteer Social Support

VNA Health Care matches you with a volunteer who will supplement the work of professionals and support staff by visiting you as a Friendly Visitor, a Caring Neighbor, Hospice Volunteer or Bereavement Supporter. Additional caregiver support and group support meetings are also available.

Wellness Clinics and Education Programs

VNA Health Care reaches out into our communities with a variety of child, adult and senior clinics offered throughout the sate, including: blood pressure, cholesterol and diabetes screenings, nutrition and development counseling; and flu immunizations.
