Home is where people feel safe. But people who are elderly and frail or others recovering from surgery, illness or injury often find it difficult to manage at home alone. VNA Health Care Independent Living Services help people live independently in the comfort of their homes rather than in a nursing home or other facility.
VNA has provided home support services for more than 100 years. Care Management Services
The VNA Health Care staff can help you or a loved one manage the complex maze of health care and support services. Care managers meet with families to assess needs, solve problems and coordinate health care. A care manager also acts as a liaison to physicians as well as to local and long-distance family members. If needed, a care manager will assist in transitions necessary for moving clients from their homes to other levels of care and can accompany patients to doctor’s appointments.
Meals on Wheels / Catered With Care
Nutritious, home-delivered meals help people who are unable to shop for groceries or safely prepare their own meals to maintain independent living at home. Our Meals on Wheels (Catered with care in the Waterbury and Southbury area) provides up to two meals a day to anyone who can benefit from the service regardless of age. The nutritionally balanced ready-to-eat meals for each day of the year are delivered right to your door and can be specially prepared to meet medical or dietary requirements.
Home Health Telemonitoring
When a patient’s medical condition requires daily or even more frequent monitoring, our in-home telemonitoring service allows nurses to monitor vital signs and other health indicators around the clock in addition to their routine visits. A nurse will intervene at the first sign of health concerns and thus reduce the likelihood that patients will need emergency room care or hospitalization.
Homemakers
A Homemaker provides socialization, runs errands, provides light housekeeping, does laundry, plans and prepares meals and much more. They also can accompany clients on medical or other appointments. Services are available to working professionals as well as those who are chronically or acutely ill. Homemakers are carefully screened and are supervised by a nurse.
LifeLine Personal Emergency Response System
VNA Health Care offers an affordable, in-home personal emergency response system that ensures that older adults or people with special needs living in their homes get quick assistance whenever it’s needed 24 hours a day, 365 days a year.
Personal Care Attendants
VNA Health Care Personal Care Attendants (PCA) can take care of your daily grooming, housekeeping and day-to-day needs. PCAs can assist with therapeutic exercise, shopping, bathing, dressing, meal preparation and eating. They can be available for as little or as long as needed up to 24 hours a day, seven days a week. PCAs are carefully screened and are supervised by a nurse.
Private Duty Nursing
When you or a loved one requires professional care from a registered or licensed nurse, VNA Health Care private duty nurses are available for shifts up to 24 hours per day. Our private duty staff is specialized in pediatric care, geriatric, hospice, maternal/child, diabetes, cardiac and wound care.
