Visiting Nurses
Visiting nurses can help you or your loved one make a smooth transition from the hospital, nursing home or skilled nursing facility to care at home. Some illnesses or injuries require special attention from nurses that have received additional training in specialized fields of medicine. Many of our VNA Health Care nurses are advance practice nurses who are certified in their fields. In addition to our regular visiting nurse services, VNA Health Care offers specialized nursing services for the following:
Diabetes
VNA Health Care diabetes nurses are well versed in the three-pronged approach to diabetes treatment and can help their patients control their disease with appropriate diet, exercise and medications. Nurses treat Type I and Type II diabetes and their related issues and provide care and guidelines for monitoring diabetics who experience acute or chronic complications, as well as those who need help with lifestyle management to avoid complications such as cardiovascular disease, diabetic nephropathy, visual impairment and diabetic foot disease.
Maternal Child
Our Maternal, Infant and Child Services provide integrated, family-centered care for pregnant women, new mothers, and infants. The agency also offers pediatric specialty services for infants and children who are medically fragile or require specialized equipment and therapies, as well as for families with a history of abuse, neglect or mental illness. VNA Health Care staff will conduct an in-home assessment and provide services in coordination with other health care professionals. Care plans are designed to match each patient's environment and level of family support.
Cardiac
Our cardiac nurses are skilled and knowledgeable in the treatment, education and rehabilitation of patients with cardio-pulmonary disease. Together with the patient’s physician, our team can implement a plan of care based on the specific health needs and individual circumstances of each patient.
Wound Care Management
Wound care nurses have received extensive training in the treatment of chronic wounds. A nurse would assess the wound, consult with the patient’s physician to develop a plan of care and provide treatment and advice regarding infection prevention, injury prevention as well as appropriate nutrition to enhance the healing process.
